The purpose of this competitive five-year program is to fund state health departments and tribal organizations to extend services to improve prevention, detection, and control of cardiovascular disease (CVD) risk factors for low-income, uninsured, or underinsured women by offering CVD screening, risk reduction counseling, referral to medical services, referral to programs, and resources to support positive cardiovascular health.
