Program objective

To improve the health of the Nation's underserved communities by ensuring continued access to affordable, accessible, and high-quality primary health care services for communities and populations currently served by the Health Center Program. Individual health center grant mechanisms include: (1) Community Health Centers; (2) Migrant Health Centers; (3) Health Care for the Homeless; and/or (4) Public Housing Primary Care. Certain funding opportunities may also expand access to primary health care by supporting new access points (i.e., new health centers and sites), expand access to high quality primary care services at existing health centers, and expand primary health care services related to emerging public health issues, including public health emergencies.

The Health Center Program provides patient-centered, comprehensive, integrated care to nearly 30 million people a year in communities across the United States through nearly 1,400 HRSA health centers that operate nearly 13,000 service delivery sites.

The Health Center Program provides patient-centered, comprehensive, integrated care to more than 31 million people a year in communities across the United States through nearly 1,400 HRSA health centers that operate more than 15,500 service delivery sites.

This program is subject to the provisions of 45 CFR Part 92 for State, local, and tribal governments and 45 CFR Part 75 for institutions of higher education, hospitals, other nonprofit organizations, and commercial organizations, as applicable. HRSA awards are subject to the requirements of the HHS Grants Policy Statement (HHS GPS) that are applicable based on recipient type and purpose of award.
